Medical Case Management

Clients meet regularly with our medical case managers who ensure access to medical care, retention in care, and update Ryan White eligibility. Medical case managers help to navigate and coordinate the complex social service system thus ensuring access to all available medical and supportive services that may help to optimize their life situations and improve healthcare outcomes.​

Non-Medical Case Management

SAAF's non-medical case managers provide guidance and assistance to clients in accessing medical, social, community, legal, financial and other needed services. Non-medical case managers may also help clients access other public and private programs for which they may be eligible, such as Medicaid, Medicare Part D, State Pharmacy Assistance Programs, Pharmaceutical Manufacturer’s Patient Assistance Programs and Health Insurance Marketplace Plans.

Mental Health

SAAF's mental health clinicians are state-licensed professional. counselors (LPC). They help clients identify goals and potential solutions to problems which cause emotional turmoil. They seek to improve communication and coping skills, strengthen self-esteem, and promote change for optimal health.

Oral Health

SAAF subcontracts with two oral health providers in San Antonio. People living with HIV who are eligible under the Ryan White program guidelines can receive referrals for oral health care. Individuals seeking services are not required to receive case management services from SAAF. Contact your case manager for a referral. Transitional Housing

Because housing stability is a crucial element in maintaining an HIV treatment regimen, SAAF provides transitional housing for up to 15 individuals experiencing chronic homelessness. The facility includes rooms for men, women, transgender and non-binary people living with HIV. Residents may remain in the program for up to 180 days and are eligible to re-enter after 90 days. SAAF’s transitional house is a substance and alcohol free environment. Participants in the program are referred from organizations providing HIV services or other social support programs, such as justice system discharge units, hospitals, or drug treatment centers. Residents are required to pay 30% of their adjusted gross income in rent monthly.

Rental Assistance

SAAF has a Tenant Based Rental Assistance program, funded through Housing Opportunities for Persons with AIDS (HOPWA), a HUD program. This program provides rental assistance for individuals and families of people living with HIV who qualify. 

Permanent Supportive Housing

In partnership with Close to Home and HUD, SAAF provides financial support for housing and social services for individuals living with HIV who have experienced chronic homelessness for 12 months.
